How to enroll

How to enroll your child at LSU

Enrolling at LSU takes four steps: tour, send an inquiry, complete Ohio's required forms, and plan a gradual first week. The only paperwork that needs an outside appointment is the medical statement from your child's doctor.

The four steps

  1. Tour the center

See the rooms, meet the teachers and ask your questions.

  2. Send your enrollment inquiry

Choose a program and start date. We confirm the opening or your waitlist spot.

  3. Complete the paperwork

Ohio requires an enrollment and health information form and a medical statement from your child's doctor. We send everything with your welcome packet.

  4. Plan the first week

Set a gradual start with your child's teacher so the first days are short and happy.

Enrollment questions

What paperwork does Ohio require to enroll in child care?

Ohio requires a child enrollment and health information form and a medical statement from your child's doctor, which includes immunization records. The center gives you the forms and keeps them on file.

How long does enrollment take?

Once there is an opening, most families finish enrollment within a week. The doctor's statement usually takes the longest, so book that visit early.
